There is an element in HTMLplus, NOTE, that serves to position
annotations. It could be twiddled a little to separate ID
and IDREF into two separate tags (as I suggested for changebars),
and Steve Putz will object to it because it's nonhierarchical
just like changebars (but these are inherently nonhierarchical
document views, Steve), but it ought to be included in discussion
of how to position annotations.
