If page numbers are a problem, why not just use <SPOT ID=#>, where # is
the page number? Then one could, with minimal source modification, make
indices point to any page of the original document. True breaks in
semantic flow (i.e. chapters) would probably be better rendered as
horizontal rules or seperate documents.
